林至芃醫師說，疼痛是一種身體存在或可能與組織損傷相關的症狀，也可能是情緒和心理因素，但在最初的評估中，首先要看的是疼痛的強度。強度可以決定，如四層敘述疼痛，如無疼痛，輕度疼痛，中度疼痛，和嚴重疼痛。
